Tiburcio Vasquez Health Center, Inc. (TVHC) is a premier provider of community-based healthcare in So. Alameda County. We provide high-quality culturally and linguistically appropriate services to diverse and low-income populations through a myriad of clinical, medical, mental health, education and support services. As part of the integrated health model of TVHC, the Behavioral Health Program provides integrated behavioral health services to patients currently receiving primary care services at TVHC. We currently have an immediate openings for a Mental Health Specialist.
MENTAL HEALTH SPECIALIST
The Mental Health Specialist is the primary interface for patients to access the program services. S/he will provide age appropriate, culturally and linguistically competent, comprehensive bio-psychosocial assessment for mental health, substance abuse, domestic violence and medical needs. Direct services will include peer/family counseling and support, screening, assessment, brief treatment, consultative services, education and engagement, linkages, outreach and advocacy. As a licensed clinician, The Mental Health Specialist will utilize Brief Evidence Based Behavioral Health Triage, Assessment and Intervention methods within a Primary Care Medical Clinic, provide linkage and referrals to other community and higher level care providers for patients. The Mental Heath Specialist will participate and provide ongoing consultation and training in mental health to primary care staff. S/he will be responsible for maintaining current listings of community resources in order to meet the needs of the target population and will develop and maintain a collaborative working relationship with medical providers. Other responsibilities are to participate in case conferences with primary care staff to provide assistance and consultation on mental health issues affecting patients and will participate in other staff meetings needed to achieve program development and evaluation. MH Specialist will be trained toward the goal of becoming certified in an Evidence Based Primary Care -- Behavioral Health Stepped Care model.
REQUIREMENTS
The position requires a Master's Degree in Counseling or Social Work. Must be a LCSW. Bilingual in English/Spanish is required. The applicant should be able to work independently and have the ability to problem solve, make decisions and handle multiple priorities. The position requires interpersonal skills with co-workers and with client families in a variety of settings and ability to organize tasks. Must possess a valid CA Driver's License, auto insurance, and willing to accept travel assignments on the job as directed by the program. The Mental Health Specialist must have knowledge of health information, demonstrate desire to work with an ethically diverse population, ability to work as a team member with co-workers and agency staff, demonstrate cultural sensitivity.
