Join a cohesive, supportive group of nurses at this dynamic not-for-profit company. The position as nurse case manager is unique from traditional nursing. We assist elderly and disabled individuals remain in their homes rather than go to a nursing home. You will assess clients in their homes to determine their level of need. We work as an interdisciplinary team with social workers, health care providers, discharge planners, and physicians. The position requires home visits and delivery of a holistic approach to caring for clients. It involves performing assessments to determine the level of need and coordinating with a social worker to obtain necessary community-based services. Other duties involve assessment for condition changes or requests from service providers or other case management team members, and relaying information to physicians and other health care partners.
EDUCATION REQUIREMENTS: Graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing. Preference is given to individuals with Baccalaureate Degree.
LICENSURE: Must have current and valid licensure by the Georgia State Board of Nursing. Current CPR certification required.
EXPERIENCE REQUIREMENTS: 2 years experience as a registered professional nurse in the area of home health, hospice, community health, long term care or geriatric nursing. Experience in long term care preferred.
JOB KNOWLEDGE: Knowledge of Medicare, Medicaid and other funding source regulations as applied to service program. Demonstrates knowledge and competence in advanced practice nursing skills, including the nursing process and collaborating with interdisciplinary team. Must be able to function independently and efficiently in community-based environment. Requires proven interpersonal skills with ability to communicate effectively. Demonstrates proven decision skills, organizational, self discipline, and time management skills. Must be skilled in physical assessment and development of care plans.
TRANSPORTATION: Must have reliable transportation with current & valid automobile insurance.
ENVIRONMENTAL AND WORKING CONDITIONS: Extensive traveling and working in patients’ homes in various conditions and environments. Highest risk is in terms of personal safety regarding on-road travel. More than one-half of the work is performed in patients’ homes.
