California Human Development (CHD) has provided services in California for more than 30 years. The corporation’s mission is “to create paths and opportunities for those seeking self-sufficiency, independence and dignity through education, training, housing, and other services.”
The Housing Department develops and manages low-income housing and other real properties in CHD service area. Performs all duties related to providing assistance and services to Parkway Plaza residents. This position contributes to the administration of facilities and properties being managed by the department.
UNDER THE DIRECT SUPERVISION OF THE PARKWAY PLAZA ON-SITE MANAGER, SERVICES COORDINATOR WILL PERFORM THE FOLLOWING DUTIES: A. Maintain detailed notes on any assistance and services provided to residents. B. Case management C. Establish linkages with agencies and services provided in the community. D. Provides information resources to project staff and residents. E. Sets up volunteer support systems. F. Sponsors educational events. G. Helps residents build informal support systems. H. Responsible for maintaining detailed records, which include any pertinent program activities and expenses. I. Keep the client files in a secured location and accessible ONLY to the service coordinator and limited access for the management. J. Must demonstrate working knowledge of supportive services and other resources for senior citizens and/or non-elderly people with disabilities. K. Must demonstrate ability to advocate, organize problem-solve, and provide results for the residents. L. Other duties as assigned.
THE SERVICES COORDINATOR MUST MEET THESE REQUIREMENTS: A. Have a High School Diploma or GED. B. Bachelor of Social Work or degree in Gerontology, Psychology or Counseling is preferable; will consider individuals who do not have a college degree, but do have equivalent work experience. C. At least three years experience in social service delivery with senior citizens and or people with disabilities. D. Have a valid CA Driver’s license and proof of auto insurance. E. Have a satisfactory driving record, as evidenced by the DMV. F. Will be required to pass a fingerprint clearance before hire. G. Experience with Resource Mobilization and Coordination Systems. H. Fiscal and operations procedures experience.
